Eligibility for Mobile Imaging Services at HOME
The MOH Mobile X-Ray & Ultrasound Authorization Form is intended for patients who medically require on-site imaging due to one or more of the following:
Patients of advanced age who require significant assistance and are unable to travel safely to an imaging facility.
Non-ambulatory patients who cannot walk or be transported without risk.
Patients with cognitive or mental health conditions that make travel unsafe, disruptive, or clinically inappropriate.

How to Complete the Authorization & Request Service:
Step 1:
A Physician or Nurse Practitioner must complete the Ministry of Health Mobile X-Ray & Ultrasound Authorization Form and fax it to the MOH Kingston Office for approval.

The Ministry typically reviews and responds within 24 hours.
Step 2:
Once approved, the Ministry of Health will fax the signed form back to you.
Step 3:
After receiving the approved form, please fax it to MD Imaging. We will then schedule a technologist for your resident.
