To utilize our services, your facility must fill out a form called the Appendix A.
The Appendix A is page 10 & 11 of an application that we must submit to the Ministry of Health and Long Term Care (MOHLTC) for approval to service your facility. The rest of the application we will fill and submit.
Only an authorized individual, such as Director of Care, Administrator or Owner etc can fill out the Appendix A.

The approval from the Ministry of Health usually takes four to six weeks.
Once the approval has arrived, you just need to fax the Requisition sheet and we will be serving your center with the required procedure.

Detention Centers can keep one or more mobile x-ray and ultrasound service providers, as allowed by the Ministry of Health and Long Term Care.
